PUPPY APPLICATION

Have you or anyone in your household ever been charged or convicted of animal cruelty or animal hoarding?
Each Prairie Paws Dogs puppy comes with a non-breeding contract. Do you agree to spay or neuter your puppy at the time recommended by your vet.
Puppies require 3 sets of vaccinations during their first year. The first set has been provided by Prairie Paws Dogs. Are you willing and able to continue the puppies recommended vaccination schedule
Prairie Paws Dogs requires a non-refundable deposit of $200. Puppies must be paid in full at time of adoption. Do you agree to these terms and conditions?
